Intraoperative and Postoperative Outcomes of Modified Bidirectional Intra-Umbilical versus Infra-Umbilical Incision for Direct Trocar Insertion in Gynecological Laparoscopy

Objectives: To describe a modified curved deep bidirectional intra-umbilical vertical incision for primary trocar insertion and prospectively compare its intraoperative postoperative outcomes with infra-umbilical in gynecologic laparoscopy. Methods: Between August 2019 March 2021, 110 patients subjected to direct technique laparoscopic intervention were classified into two groups. Group A comprised 55 cases of while longitudinal was used group B (55 cases). Intraoperative assessments performed. Results: There statistically significant increased numbers parity, gravidity, previous cesarean sections; smaller number infertility complaints B. Likewise, expressed less peri-trocar CO2 leakage (46 patients, 83.6% versus 28 50.9%) more tightness the portal entry (45 81.8% 30 54.5%) if compared throughout whole operation. On follow-up after one month, there (p-value = 0.029) decreased OSAS PSAS (10.4 ± 4.2 11.8 4.3) i.e., better cosmoses when (13.3 5.7 16.0 6.8) respectively. Conclusion: Performing is simple fast step that results elimination gas slippage without need any additional sutures. Aesthetically, it scar satisfactory incision.
